Lomustine, also known as CCNU, is an N-nitrosourea derivative with potent antitumor activity. It is a pale yellow powder that serves as an alkylating antineoplastic agent. Lomustine is used in the treatment of various types of cancer, including brain tumors, lung cancer, malignant melanoma, and other solid tumors. It is available in 10-, 40-, and 100-mg capsules for oral administration and is well absorbed, widely distributed, and capable of crossing the blood-brain barrier. Lomustine undergoes extensive hepatic metabolism, mediated by CYP3A4, resulting in several hydroxylated metabolites, some of which are more active than the parent compound. The drug is primarily eliminated in the urine with an elimination half-life of 16 to 72 hours.
